We are proud to offer the opportunity for our customers to provide employees with the Remodel Health Reimbursement Program! Because our program is in-house, it allows employers to have more flexibility and control over employee reimbursements, and it provides convenient payments for employees.

All employees that opt in to coverage are eligible, so everyone stays happier and healthier!

Navigate to your personal Remodel Health member portal at app.remodelhealth.com.

Step-by-Step Guide

Follow these instructions to successfully submit a reimbursement request to your employer!

Go to your My Benefits tab on the lefthand side ribbon.

Click Start to begin your Reimbursement Program submission.

Provide your basic personal information in the fields below Employee Information.

On this page, enter how many expenses you will be submitting as well as the type of expense.

Scroll down to Team Benefits and look for Reimbursement Program - Remodel Health.

Here, click Submit Expense.

When you are ready, select Next.

Complete the submission form according to each field.

Please note, if submitting a medical reimbursement, the expense file MUST be an Explanation of Benefits (EOB) from your major medical carrier. 

Read the Statement by Participant, select the Date, and Sign before final submission.

Congratulations! Your reimbursement request has been submitted. You will receive a bounceback email from Formstack notifying you of submission. The Remodel Health team will begin reviewing your request and will let you know if anything additional is needed.

When you are ready, select Next.

When you are ready, select Next to navigate to the final page.

Finally, click Submit Form to submit your reimbursement request.
