What is an ICHRA?

An ICHRA is a tax-advantaged health benefit that enables employers to offer employees a monthly contribution amount to pay for individual health insurance premiums. Depending on the plan design, employers can also offer an HRA to cover qualified out-of-pocket medical expenses.

With an ICHRA, you set your preferred contribution amount according to your budget. With no annual caps and the ability to vary contributions based on age, family status, or employee class, the ICHRA has powerful customization options. Participating employees then enroll in a qualifying individual health coverage and pay for premiums with their ICHRA funds.

ICHRA contributions are tax-deductible and payroll-tax-free for employers and free from income tax for employees.

What size companies does Remodel Health work with?

Remodel Health works with organizations of all sizes, primarily serving large and enterprise organizations with more than 100 employees.

Who can offer an ICHRA?

Any business owner can offer an ICHRA to W-2 employees, regardless of the company’s size, industry, or location.

Does the ICHRA have any contribution limits?

The federal government doesn’t set minimum or maximum contribution limits on the ICHRA. The employer chooses how much they’re willing to contribute to an employee each month. Employers can also vary contributions by employee classes, age, and family status.

Who can participate in an ICHRA?

Only full-time or part-time W-2 employees and their eligible dependents who have qualifying individual health coverage that provides minimum essential coverage (MEC) can use the benefit.

Does COBRA apply to ICHRAs?

Yes, the ICHRA is subject to COBRA requirements.

Are ICHRAs compatible with HSAs?

Yes, but the ICHRA must have a specific plan design to coordinate with a health savings account (HSA). Employers must offer a limited-purpose ICHRA to employees with an HSA that restricts reimbursements to premiums and permitted excepted benefit expenses such as dental, vision, and long-term care.

Who owns an ICHRA?

The ICHRA is employer-owned. Unused contributions stay with the employer at the end of the plan year or if an employee leaves the company.

Who can contribute to an ICHRA?

Under federal law, only employers can fund ICHRAs.

Can an ICHRA satisfy ACA requirements for applicable large employers?

Yes, as long as the allowance meets IRS affordability standards and the employer offers the ICHRA to at least 95% of full-time employees and their dependents. For 2026, coverage is affordable when an employee’s cost for the lowest-cost silver Marketplace plan, after the ICHRA allowance, is no more than 9.96% of their household income. 

Employees must also enroll in a qualifying plan that meets MEC and minimum value standards.

What does an ICHRA cover?

Depending on plan design, ICHRAs can cover individual health plan premiums and more than 200 types of qualified medical expenses outlined in IRS Publication 502 and the CARES Act.

Does offering an ICHRA have tax advantages?

Yes, ICHRA contributions aren’t subject to employer payroll taxes and are exempt from employees’ taxable income.

Are there any reporting requirements for an ICHRA?

Yes, but reporting requirements depend on the company size. Employers with fewer than 50 FTEs file Forms 1094-B, 1095-B, and Form 5500-SF or 5500-EZ if applicable. Employers with 50 or more FTEs file Forms 1094-C, 1095-C, and Form 5500 if applicable. They also must file Form 720 and pay the Patient-Centered Outcomes Research Institute (PCORI) fee.
